A champion losing without getting pinned is an affront to everything that's supposed to be appealing about wrestling. I feel like that door getting kicked in in the '90s led to a lot of other, "The rules don't matter, pro wrestling can be ANYTHING!" type booking that I stand with Jim Cornette on.
Ric Flair did not say, "To be the man, you've got to beat the man or at least beat another man in the same match as the man."

You can always do an elimination three way match so that there's a conclusive winner. The "champion can lose without being pinned" thing has always bugged me. It's a lame cop-out finish every time.

In real-life, I absolutely DESPISE the three-way dance for the exact reasons already mentioned... Because 90% of the time they're used to facilitate a cheap finish.
BUT... For the purposes of the game, I love booking the occasional Triple Threat match because I can spin the conclusion however I see fit... Sometimes, the dice might decide that the champion will actually eat the fall - which is a bonus... If not, it provides me with an opportunity to tell a story which otherwise might not have taken place... Since we are the promoters, we can decide to use this match stipulation in a manner that benefits the audience.

The worst issue with the triple threat to me is you watch someone hit a finisher only for the third wrestler to score the pin. Yes, it's just lazy booking and a crutch most of the time.

Triple Threat or Fatal 4ways with one fall are okay to determine a number one contenders but would only use elimination rules match fkr title matches. Like others, I am not a fan of losing a title without getting beat.

I think triple threats and four way matches are fine, but sadly used to much in real life wrestling. I like Rob and Pariah's post about the subject. I think they need to be special like an attraction among main eventers to put butts in seats.

I don't think I have ever done a 4-way dance...
BUT, for 3-way dances, Each participant has to be pinned TWICE. (What?) Well, it's really a series of 1-on-1 matches.
Wrestler 1 vs. Wrestler 2 (part 1 of match) Winner of part 1 takes on Wrestler 3 (part 2 of match) Winner of part 2 takes on loser of part 1 (if loser of part 1 loses for second time, then that individual is eliminated) (part 3 of match) Winner of 3 takes on loser of part 2) (this will be a participants second loss, that individual is eliminated) (part 4 of match) IF NECESSARY, Winner of part 4 takes on loser of part 3. (this will be a participants second loss, so that individual is eliminated)
The match can end in part 4 or 5...just depends on when second losses happen.
It really isn't that confusing. 2 start, winner stays...loser waits...winner stays...loser waits...two losses and you are eliminated. Oh yeah...no DQ's nor count outs.
