Coming out of Retirement Chart?

So, I am playing a modern day (or close to--2020 and catching up) fed, and I have an LOW guy who is coming out of retirement for one match. I am looking everywhere, but I cannot find the "Coming out of Retirement" chart.

Can someone direct me to this chart (it's in one of the official handbooks), and/or with the moderators' permission, could you just post or PM me the adjustments for a guy who's been retired for 6 years?
