I got Mayhem in the mail today, and I don't have any booklets anymore, so I don't remember how the Leap of Doom worked. Can someone explain it please? I assumed it was a roll finisher somehow, but I don't remember. Thanks for any help.

It's a roll finisher yes...hits of 4,5,6. IIRC, the opponent takes over on lvl 3 offense on the missed, but someone else should who has the book in front of them might want to confirm

Yep, Wildfire is correct. Maybe the original Extreme finisher in the GWF
Now that I look back on it, if Mayhem hit the move, he got to add that many to the PIN rating. I wonder if Tom had to do it over, if he missed, Mayhem acquired that many pin tokens (1, 2 or 3). That would have really made it an extreme move
