For those who do them...
How do you start the falls after the first fall?
Do you start with the winner of the 1st fall on Level 2 or 3 offense? Do you just start back with the winner of the first fall on Level 1 Offense? Or something else?

Level 2 personally. They're on a roll but the opponent has had a little time to rest.

Depends.
I re-roll for inititive with the winner of the last fall with a + to their roll. Played by ear. And unless the first fall took forever; I'll start back at lvl1 offense.

Winner of the fall starts on lvl. 2 offense. If a tag match, the same legal men start the next fall.

In my Fed I imagine there's a 'rest period' between falls which allows for even the fatigued wrestler to recover. I like to build a match pace mechanism into the mix. I use a rule which makes it more difficult for the wrestler who dropped the fall to start...but not impossible....while also staggering at what offensive level the wrestler who won the prior fall can take over at. Initiative roll for the next fall after a pin/submission uses a straight up 1D6: i. If the wrestler who won the prior fall has the higher 1D6 roll...he starts on Level 2 Offense. I imagine this reflects damage was done, the winning wrestler has momentum and the wrestler who dropped the first fall is too fatigue to react at a Level 1 Defensive Level. ii. If the wrestler who was pinned has the higher 1D6 roll, then I re-roll for him again but this time use a 2D6 with his Pin plus fatigue as the gage. If he wins that roll then he takes over on Level 1 Offense. If he fails it just means that the other wrestler starts on Level 1 Offense. Momentum levels are more even in ii. than in ex. i. By restricting the losing wrestler to a start point of Level 1 it builds a consequence into the prior fall while also keeping a random outcome dynamic alive for how much momentum the winner of the prior fall actually has going forward.

I use the "rest period" idea. The winner of the first fall gets the advantage to start the next fall, but I also will (depending on how man tokens) start both guys with tokens...obviously with less than they had (most of the time 0). It shows the fatigue. I also use the idea of "great heart" with pinfalls...especially in the third fall!
