When I am rolling a vintage match, which would include guys like Jack Brisco, Buddy Rogers, and Lou Thesz as well, I typically use 2 out of 3 falls for all title matches. One fall for non title matches. It just seems right.

Several promotions used 2/3 falls for almost every match even into the early 1950s. I think one of the reasons was many shows were only 3-4 matches long and this gave the card some length.
Regarding the injury result, there is no "official" rules and promotions differed. I would probably give the uninjured wrestler the fall and record it as his opponent was unable to continue.

Don Owens Promotions (Portland Wrestling) still utilized 2/3 falls for the Main Event and especially Title matches through the 80's and early 90's when Don finally retired and sold off the business... And i have to admit, we enjoyed the hell out of it, lol... now it was booked not dice controlled, so there is a HUGE difference (I say that hopefully very sarcastically, lol) and it was obviously used to push things forward to make folks look better (I got to see Brett Sawyer submit Ric Flair with a Figure Four in the second fall of their televised match... believe me, we popped and marked like prepubescent girls for a boy band)...
I think the 2/3 falls is highly underutilized and sinfully under appreciated nowadays, because of the shorter attention span of most wrestling fans and the need to get every possible high-spot and set of moves/reversals/counters/no-sells/absorb and takes/etc as possible into a 10 minute match and it just doesn't translate well to a lengthy 2/3 falls match... The flip side being on the uber rare occasions they occur, IF (IF, IF, IF) they are done right, they are sold as "the amazing stamina of these people" and actually go over pretty well...
On the injury question, I think Tim has the easiest and simplest answer for the day to day usage... however on that rare occasion you need to "show someone's heart, etc.), then I agree with Cory, use the injury chart (though i would use the worst setting on the chart regardless of the length to represent the "freshness" of the injury, just my thought)

In my Anarchy in the UK (Legends) fed I have best of 3 falls in 10 Rounds rules

Thanks for all the help and ideas. I like using max injuries if the wrestler chooses to continue. Muldoon has a title defense against Turk in a couple of weeks. If the match doesn’t end decisively, then the rematch will be 2/3 falls.
I’m also toying with the idea of Pesek winning a midcard title and defending against opponents he hates (backstage bullies, drunks, etc.) Pesek would either win the first fall or lose by dq, then either win by ff or destroy the injured foe. It’s not completely honorable, but I’m playing Pesek as more of an antihero.
