I read in Shooters that many, if not all, matches back during the Vintage era were 2 out of 3 falls, and I’m debating on whether to use that in my fed. Currently I use the vintage charts for Vintage Match rules whenever wrestlers of that era fight, though they can choose to use normal rules or the GM can order the match, just like a cage or Texas death match.
What I’m wondering is, should 2/3 Falls be done for all Vintage matches, title matches, or just for special occasions? What do you think?

I tend to use them for all Title matches/main events... i also use them every time for any dice/chart enforced "rematch"... my original goal had honestly been to use them for every match (still a beautiful dream, lol) but with the limited time i have to actually play anyways, it seemed unrealistic

All of my tag team championship matches have always been best 2 out of 3 falls. But I rarely do best 2 out of 3 falls for 1 on 1matches.

In my Japan fed all title matches are 2/3 falls.

That's a great question, Shon Maxx. For me personally, I save it 2/3 Falls as a stipulation, usually for guys who have traded wins in a series of matches and who are more technical wrestlers than brawlers. Obviously, it's up to you, how often you want to use it. But if you are a fan of the matches, I would say using them in your World title matches would make sense.

I might save it for title matches only, or even just ppv title matches. Next question: In Shooters, more than once a wrestler won the first fall by pin or submission, and the next by forfeit because the opponent was injured and couldn’t continue. So what happens if a wrestler wins but gets injured? For example, Bob Roop is defending the midcard title against John Pesek. Pesek, in Enforcer Mode, gets disqualified for the first fall, but Roop is now injured 6 cards. Does this result in Pesek winning the second fall, resulting in a draw, winning both the second and third falls by ff, or something else?

I personally would play each match/scenario differently, depending on who was involved. I could see having Roop gut it out with adding fatigue and decreased abilities from the Injury Table, maybe for one fall and then forfeiting the third fall. But I could see having Roop forfeit the last two falls.

2/3 falls matches were definitely the most common but another type of match that was fairly common was what they called a handicap match, which are not at all the same as modern handicap matches. The vintage style of handicap matches would have a high ranked wrestler taking on a much lower ranked wrestler. The stipulation would be that the higher ranked wrestler would need to pin the lower ranked wrestler a specified number of times within a certain time limit. So, using guys from our game, Frank Gotch might say he can pin Henry Irslinger 3 times in an hour. If Gotch didn't achieve that, Irslinger would win and would get the larger portion of the purse.
To answer your specific question though, I save 2/3 falls matches for special occasions for my fed. I think if I were to have someone like Frank Gotch win the title, I might have him make that a stipulation for all of his defenses.
