So I was watching the match in the legends tournament earlier today and was looking at the mechanic.
Does his mechanic of submission then add one and pin if unsuccessful mean that 1, 2, or 3 tokens are added if neither work?
It could be read any of those ways and make sense depending on who was doing the reading.
1 = Just follow the instructions on the card (this is the least likely in my opinion, since it ignores all the failed finish attempts). 2 = 1 token for failed submission (this is the add one in the mechanic instructions), 1 token for failed pin. 3 = 1 token for failed submission, add 1 as per the instructions, 1 token for failed pin.
Personally I always assumed it added 2 tokens 1 for each of the failed finish attempts...but I figured I'd ask what other promoters do with his mechanic, or if there's an official way it's supposed to be played.

Yes, this is correct. If you play Champions of The Galaxy, it's the same mechanic that Anarchy (and later Total Anarchy on Centra) had for his finisher.
