I roll 3x's for my weekly shows w/o the advanced PIN rating, so that the matches don't last too long and then I use the advanced PIN for Supercards and still roll 3x's to simulate the lengthier matches you would tend to get with those. That's an interesting statistical question in the 1 roll against an advanced PIN Rating versus 3x roll for standard PIN Rating. Which initially I would imagine isn't to noticeable w/o fatigue, but I imagine it's quite noticable 1 to 2 fatigue tokens in, when it comes to the margins. I think we need get a statistian or a retired accountant to break those margins down. Hmm I wonder where we could find one on the boards ;P

Not exactly the same thing, but the probability of rolling any number on a D6 is 1 in 6, the probability of rolling that same number two consecutive times is 1 in 36 and the probability of rolling it three consecutive times is 1 in 216. The odds against consecutive successful rolls increase exponentially.

I haven't done any "tracking" of the number of rolls, but I have found that w/o the advanced PIN ratings on my weekly show, the matches don't tend to last too long. Especially with fatigue factored in. A guy with a 7(4) gets 1 or two tokens and it might be a 6-8 minute match. Maybe I am doing it wrong? I don't roll for the exact PIN number, I am rolling for the PIN number and below. So if a 7(4) acquires two tokens, then he's at 9 - so now I'm just rolling to get 9 or lower 3x's in a row.

I just roll 1 time for PIN except for when a current champion is being pinned in which case I roll PIN twice.
It has helped lead to some impressive title reigns for example Aethran Overmaster holding my ROV Championship on his Reign of Terror from Show #73 through Show #100 and the Predators setting a tag team record by holding my ROV Tag Team Championship from Show #84 through Show #111. At the same time, short title reigns are also still possible as, for example in my ROC fed, Prince of Rose recently only held the Carnage Championship for 1 show.
I do also roll twice for DQs.

I'm pretty sure that the Fatigue Token rule is meant to be used with the Advanced PIN Rating (the number in the parentheses), because they are both Advanced rules. Otherwise, as you have pointed out, the matches could hardly take any time at all. That said if that system works for you, go ahead and use it!

I just roll pins once. At times over the years, I have flirted with rolling twice for defending champions, but have ultimately decided that I like the flukiness and chaos that rolling once can bring sometimes. I did start rolling DQs twice for chart DQs after seeing people mention that on these forums and that has been a very nice change.
