Does anyone else print out PDF bootlegs? How does everyone else do it, and what options do you have if you want quality printed cards? -Do you prefer to print them at home? -Do you go to a print shop? -Are there any online options?

I'd never print them at home. Most home printers can't handle the cardstock that is required to make quality color cards.
I take mine to a print shop in town. Each page of 2 cards costs about 3.40 each including sales tax. There's also additional fees for things like small orders, overly large orders, same day, cutting, etc. So the final price really depends on a lot of factors.
As far as options, I do my cards on 100 or 110 lb. cover stock. Make sure you DON'T let them use 110 index. It worked fine for print at home black and white cards, but it's very flimsy and will feel very underwhelming when compared to the official color cards.

I also get mine printed at a local print shop. I tell them:
4/4 silk 111# 4.25 x 5.5
I bought them in an official FG card and they guy said those are the stats. I think, like Carlzilla said above 111# is the cover stock and they have a silk finish. 4/4 is full 4 color I believe. Anyway, whenever I take an order in I just tell them that and they come out great.

In the black and white days any decent home printer could make cards basically indistinguishable from the real thing. I did literally hundreds of those. For color I just use a premium 110 lb card stock and the highest quality print setting on a good printer. It does the job but you can tell the difference compared to the official printed cards. If you want the closest to the real thing these guys are on the right track. You'll never get the finish right on a home printer.

Office Depot does mine. I get a discount with my union benefits and it works great. That's where I have printed like the old B&W PDF and print only sets, as well as the pro prime cards, note only those that I will use - from 2014 & 2015.
Cost is relatively inexpensive with them. However, the discount is pretty great on them, through my union benefits.
