I may start rerolls for initiative but begin on L2 to show continuity of this as one match. Most of these matches from 85 on (especially WWF) the subsequent falls were shorter than F1 so I like the idea of shorter later falls.
For tag matches usually hold off saves until a team is in serious danger of losing their 2nd fall.
Too bad there are no official rules.

I roll for initiative before each fall. I don't add a fatigue token for the winning fall of the previous fall but I carry over the rest of the fatigue the same. Realistically, I should be carrying over that one but I look at it from the pro wrestling booking side of things, that the rest period between each fall gives the losing wrestler some time to recuperate a bit. I hope that makes sense!

I didn't reduce any fatigue, as the rest period between falls is nominal in modern matches. However, if using the Pioneer Era wrestlers, I would probably have to re-think that. Rest periods were much longer and the wrestlers often returned to the locker room to rest between falls. As far as initiative, I roll to start each fall.
And in tag-team matches, the legal wrestlers at the end of one fall have to start the next fall.

Just keep going the pins. After 1st fall I start again from beginning, no level 2 offence in mine.

Seems like an even mix of initiative and how fatigue is handled as I carry over all fatigue as there really isn't much rest between falls, but to account for the 'rest' I do a reroll of initiative to start at L1 offense.
