How often do (ag)/(pw) moves even come up once the match is going? It's not never, especially if there's one on L2O #2, but as mentioned...not a ton of the match takes place on Level 1 Offense to begin with.

On doubles I roll on an announcer, backstage, or "segment" chart.

Yea you could do that and you’ll be rolling more and making matches longer. I like this house rule but it makes the matches longer since you spend more time on LVL 1. The original rules deter going back to level 1 offense unless one wrestler has the advantage. Would you really be on L1 more, though? (ag)/(pw) moves never follow a 1 move and if it's countered, the opponent takes over on L2O. So you're rolling on L2O (on an unsuccessful move) or L2D/L3D no matter what. I see what you’re saying. My thought process was instead of going to a (c) chart, you would roll up. Unless you had a level 3 offensive move on LVL 1 OFF or the advantage in both ag/pw it’s ill advised to roll up. So each time you do it’s a time you wouldn’t have rolled on LVL 1 OFF then you’re rolling again to see if the move works. In addition to that You could end the match off the (c) chart or end up on LVL 3 OFF or LVL 3 DEF thus shortening the match.
